1. Who is responsible

Dr. Sara Zapata Aesthetic Dentistry, located at Av. 6A Bis #35N-100, Suite 402, Centro Empresarial Chipichape, Cali, Colombia, is responsible for the personal information submitted through this website.

2. Information we collect

The general contact form collects only your full name, email address, WhatsApp number when supplied, country and city, chosen contact language, preferred contact method, and authorization to process those details. A WhatsApp number is required only when WhatsApp is selected as the preferred contact method. We may also process limited technical information required to protect the form and WhatsApp link against fraud and automated abuse.

3. How we use it

We use this information only to answer a general commercial enquiry, explain how to begin, and coordinate a later conversation. We do not use the public form to establish a diagnosis or treatment plan, and we do not sell personal information.

4. Authorization and service providers

By selecting the consent box and sending the form, you authorize the practice to use the submitted contact information for the purposes described above, under applicable Colombian personal data protection rules. The website validates the request without saving it to its application database, and the visitor’s browser then sends the commercial fields to FormSubmit for delivery to info@doctorasarazapata.com. According to FormSubmit’s current public documentation, complete submissions are available in its archive for 30 days. FormSubmit and the clinic’s email provider operate under their own terms and privacy practices and may use infrastructure outside Colombia. Form data is not sent to Meta for analytics or advertising.

5. Sensitive health information and later clinical communication

The public form contains no open-text, treatment-selection or file-upload field and must not be used for health information. If photographs, radiographs, symptoms, medical history or treatment details are needed, the clinic will request and manage them later outside ChatGPT Sites through the channel it determines. Do not send that information through this form and do not use this website for an emergency.

6. Cookies and privacy choices

The website uses necessary technologies for security, language selection and remembering your privacy choice. Optional analytics and advertising categories remain disabled unless you choose them. You may accept, reject or change those optional choices at any time through “Cookie preferences”. Meta Pixel and Conversions API are not currently active.

7. Retention and security

FormSubmit states that it retains complete form submissions for 30 days. The clinic keeps the delivered email and related general contact information only for as long as reasonably necessary to respond, protect its channels or meet applicable obligations. We use reasonable administrative and technical measures, but no internet transmission can be guaranteed to be completely secure.

8. Your rights

You may ask to know, update or correct your information; request evidence of your authorization; ask how it has been used; revoke authorization or request deletion when legally applicable; and submit a complaint to the competent Colombian authority after using the practice’s enquiry channel.
